US Club Soccer Form R002 Instructions

Spokane Rapids registers all players with US Club Soccer to provide player eligibility and accident insurance coverage during participation in club activities. As part of this registration process, US Club Soccer requires every player to have a completed Form R002 on file. 

 Form R002 includes player information, a medical treatment authorization, and a liability waiver, and must be completed by a parent or legal guardian.

Why This Is Important

Players who do not have a completed Form R002 on file are not considered registered members of US Club Soccer, even if membership fees have already been paid.

 Without a completed form, players: 

  • Cannot participate in practices, games, scrimmages, or tournaments 
  • Are not covered under US Club Soccer accident insurance 
To ensure all players remain eligible and insured, please complete this requirement as soon as possible after receiving your registration email.

There may be a delay between when you register your player with Spokane Rapids and when your player is fully registered with US Club Soccer. Once the US Club Soccer registration process has been completed, you will receive an email with instructions for completing Form R002. 

How to Complete Form R002

  1. Log in to your GotSport account 
  2. Navigate to your player’s account and find available forms 
  3. Complete and submit the US Club Soccer Form R002
